A survey is a simple and effective way to identify any gaps in knowledge so that you can target education where it is needed rather than making assumption. The survey for children and the survey for parents have some similar questions so that you can make a comparison between the answers.
Instead of using paper, you could consider using a free online tool such as Survey Monkey or even using a Google form so that the results are compiled automatically for you.
Before you download, please note the following:
- Tailor the questions to your own unique circumstances.
- I’m no graphic designer, please don’t expect something pretty.
- I haven’t put anything like, “Have you ever been cyberbullied?”; If you want to put questions like that you MUST get clearance from the Headteacher and governing body. The reason being, if you are made aware of such issues, you have a duty of care to act upon them.
If you want the children to complete the survey electronically you will need to “Protect” the document in MS Word so that only the Form elements can be altered.
